The town takes its name from the natural limestone cave and mineral spring at its center, and that spring has shaped the community's identity from the start: it flows from the cave into a rock holding pond in Rolater Park and then into a spring-fed lake, and it still supplies clean drinking water to the area and draws visitors who come to fill jugs at its source. The town's history is unusually rich for its size. Rolater Park's grounds once served as school property associated with the Cave Spring Baptist Church, and the area hosted Hearn Academy, a notable early academy whose 1910 building and former dormitory (now the Hearn Inn) still anchor the town. During the 1864 Atlanta Campaign, both Confederate and Union troops used Cave Spring for hospitalization and rest. In 1931, Dr. J. B. Rolater deeded the 29-acre park — cave, spring, and pool — to the town, and the area is now listed as the Rolater Park Historic District on the National Register of Historic Places.
